We have provided two forms below. Teachers registering for their students should only use the "Teacher Registration" form and register all their students at once. The "Student Registration" form is only for students registering alone and independently.
In order to participate, teachers must register their students one week before the recital date. We collect a registration fee of $5 per student. Teachers can either pay in cash or mail a check with the total fee payable to the Alzheimer's Association. Checks should be mailed to MSSL Registration at 117 Sabiosa, Irvine, CA, 92618.
Students that register individually must register two weeks in advance, fill out the separate form below, and bring either $5 in cash or a check for $5 payable to the Alzheimer's Association.
These fees are donated to the Alzheimer's Association at the end of the season.
